Driver's license, state ID, or passport for the participant.

Insurance card(s)

Medicare, Medicaid (incl. LTC waiver), Medicare Advantage, VA, or commercial — front and back.

Medication list

Current medications including name, dose, frequency, and prescribing physician. Original labeled containers required if brought to the center.

Primary physician information

Name, practice, phone number, and most recent visit date.

Emergency contact information

At least two contacts with phone numbers and relationship to the participant.

Power of Attorney (POA)

Durable POA documents if a family member or representative makes care decisions.

Healthcare surrogate / advance directive

Designated healthcare surrogate forms, living will, or DNR if applicable.

Diagnosis & medical history summary

Dementia / Alzheimer's stage, Parkinson's, diabetes, cardiac history, etc.

Mobility & equipment information

Wheelchair, walker, cane, oxygen, hearing aids — and any transfer instructions.

Therapy notes (if applicable)

Recent PT, OT, or speech therapy notes.

Dietary restrictions & allergies

Diabetic, low-sodium, vegetarian, no-pork, thickened liquids, feeding-assist needs.

Authorized pickup list

Names and phone numbers of every adult permitted to pick up the participant.

Program Assessments

Every participant is assessed before enrollment.

The pre-enrollment assessment determines Standard vs Enhanced Support (Level I, II, or III) placement, transportation eligibility, dietary accommodations, self-administered medication-reminder needs, and the daily non-medical structure best suited to your loved one. Assessments are held at the center or in-home for mobility-limited adults.

How long does enrollment take?

Most families move from first call to first day in 5–10 business days: tour (Wed/Thu/Fri), intake application, pre-enrollment assessment, and enrollment confirmation. Faster timelines are possible when documents are ready.

Who is eligible for Adult Day Care?

Adults 18+ who benefit from structured daytime supervision, socialization, meals, and activities — including seniors with dementia, Parkinson's, stroke recovery, or general aging concerns. A pre-enrollment assessment determines Standard vs. Enhanced Support (Level I, II, or III) placement. Boca Raton Adult Day Care Center is a licensed social adult day care center and does not provide skilled nursing, therapy, medication administration, or medical treatment.

What documents do I need?

Photo ID, insurance card(s), current medication list, primary physician contact, at least two emergency contacts, POA/healthcare-surrogate documents if applicable, and any recent hospital or therapy notes. A full checklist is on this page.

Do you provide transportation?

Yes — within our designated service area around 2120 N. Dixie Highway, Boca Raton. Eligibility is based on route, mobility, schedule, and assessment. Use the calculator or transportation page to preview your route.

How does the pre-enrollment assessment work?

A trained team member reviews medical history, mobility, cognition, medications, and family goals. It happens at the center or in-home for mobility-limited adults, and sets the care plan before day one.

Do I need to tour before enrolling?

Yes — a tour on a Wednesday, Thursday or Friday (11 AM–3 PM, 48-hour advance notice) is required. It protects your family from an uninformed decision and lets us match your loved one to the right program.

What payment options do you accept?

Private Pay, Long-Term Care insurance, Medicaid LTC, and VA benefits (subject to eligibility). Credit card payments include a 6.5% processing fee. No card numbers are collected on web forms.

Are payments refundable?

Program fees, registration fees, and transportation fees are non-refundable; the $250 Behavioral Deposit is also non-refundable when applicable. Cancel 48+ hours before a scheduled day and you receive an account credit (no cash refund) for a future day, subject to availability. Cancellations under 48 hours or no-shows forfeit the day with no refund and no credit. Adult Day Care requires a 3-day-per-week minimum.

What are the one-time enrollment charges?

Every new participant pays two one-time charges at initial enrollment: a $75 Registration Fee (non-refundable; covers intake, records setup, and administrative processing) and, when applicable, a non-refundable $250 Behavioral Deposit (applied only when required based on assessed needs and enrollment terms; helps cover extraordinary cleaning, property damage, or supervision needs).
