Program Assessments

Every participant is assessed before enrollment.

The pre-enrollment assessment determines Standard vs Enhanced Care placement, transportation eligibility, dietary accommodations, medication-reminder needs, and the daily structure best suited to your loved one. Assessments are held at the center or in-home for mobility-limited adults.
