Comprehensive Care Assessment
A unified intake covering ADLs, IADLs, cognition (Mini-Cog + AD8 informant), behavioral profile, fall risk, diet, home environment, and program triage. Used to recommend Program A, B, or C and assign the correct care tier.
IMPORTANT — Please read before completing:
- This assessment is not a medical diagnosis. It informs our internal care recommendation only.
- Program A (ADCC) requires a 3-day-per-week minimum.
- Program B (In-Home Care) is Palm Beach County only, with 8-hour minimum shifts and a 3-day weekly minimum.
- Program C (Wellness Clinic) is secondary support, Tuesdays/Thursdays only, not a replacement for PCP, urgent care, or 911.
- Submissions are routed to the care team at PFAdultDayCareCenter@gmail.com with delivery confirmation logged.
