A unified intake covering ADLs, IADLs, cognition (Mini-Cog + AD8 informant), behavioral profile, fall risk, diet, home environment, and program triage. Used to recommend Program A, B, or C and assign the correct care tier.

Program A (ADCC) requires a 3-day-per-week minimum.
Program B (Boca Raton Homemaker & Companion Services) is non-medical only (AHCA #242034) and Palm Beach County only, with 8-hour minimum shifts and a 3-day weekly minimum.
Program C (Wellness Clinic) is secondary support, Tuesdays/Thursdays only, not a replacement for PCP, urgent care, or 911.
