Step 5 — Pay your reservation deposit

Pay online once your slot is confirmed

A $75 deposit is collected when the appointment is confirmed and is credited in full toward your visit total. The balance is due at the visit. We never store card numbers, CVV or banking credentials — payments run through our authorized processor with tokenized cards. Your receipt is emailed immediately and posts to your clinic account.

Credit, debit, HSA or FSA card. The deposit is credited toward your visit total, not added to it.

Card details are entered on the payment processor's secure form — never stored by the Center.

Insurance

Insurance coverage depends on the treating provider's credentialing and participation status, the patient's individual plan, eligibility, benefits, authorization requirements, and the specific service provided. Coverage is never guaranteed until verified.

Patients may submit their insurance information before the appointment so our team can complete the applicable verification process.

Clinic patients drive themselves or come with family — door-to-door transportation is an Adult Day Care (Program A) service and is not included with a clinic visit. Use the checker to see your distance and plan your arrival, or choose a telehealth visit where eligible.

Eligibility is never automatic. Final approval depends on route review, mobility assessment, wheelchair/transfer requirements, vehicle capacity, and program enrollment status.

Payment, cancellation and fees

  • A $75 deposit is collected when the appointment is confirmed and is credited in full toward your visit total. The balance is due at the visit.
  • A $45 fee applies to a no-show or a cancellation made less than 24 hours before the appointment.
  • Every visit is by appointment. We do not accept walk-ins, so each patient gets the full scheduled time.
  • We are a supportive wellness clinic that works alongside your primary care physician. We do not replace primary care, and we are not urgent care or an emergency room.
  • For chest pain, stroke symptoms, severe bleeding, trouble breathing, or a fall with injury, call 911 immediately.
