Transportation is an optional add-on for approved Adult Day Care participants only. Available when the verified pickup/drop-off address is generally within our designated service area surrounding our facility at 2120 N. Dixie Hwy, Boca Raton, FL 33431. Eligibility is never automatic — every address is subject to route review, mobility assessment, capacity, and final approval.

Transportation
Three transportation levels — priced by the assistance actually needed.
Door-to-door transportation is an optional add-on for approved Adult Day Care participants, billed as a separate daily line item and priced by the level of physical assistance the participant actually needs — never by guesswork. Your level is confirmed at the pre-enrollment assessment.
Level I
$35 /day
Ambulatory — round-trip
For participants who walk independently or with a cane and can board the van using the standard step with light supervision.
- ✓Curb-to-curb pickup in the morning and return in the afternoon
- ✓Uniformed driver escort from the door area to the van seat
- ✓Seat-belt check and headcount before every departure
- ✓Family text confirmation on arrival at the center on request
Level II
$45 /day
Walker / mobility device — round-trip
For participants who use a walker, rollator, or need steady hands-on assistance to board, sit, and rise safely.
- ✓Hands-on steadying assistance at the curb, step, and seat
- ✓Walker or rollator folded, loaded, and secured for the ride
- ✓Extra boarding time built into the route so no one is rushed
- ✓Same driver and route team whenever scheduling allows
Level III
$55 /day
Wheelchair / transfer assistance — round-trip
For participants who use a wheelchair or require lift access and two-person assistance to travel safely.
- ✓Hydraulic wheelchair lift with a rated, inspected platform
- ✓Wheelchair secured with a four-point tie-down and occupant belt
- ✓Two-staff assistance at pickup and drop-off when required
- ✓Coordination with the family or facility staff at the home door
All rides are round-trip and available only to approved Adult Day Care participants whose verified pickup and return address is within our designated service area around 2120 N. Dixie Highway, Boca Raton, FL 33431. Eligibility, level, and scheduling are subject to address and route review, mobility assessment, vehicle availability, staffing, and final program approval. Transportation is billed separately from program tuition. Payments are non-refundable; please give 48 hours notice for schedule changes.

Our fleet is made up of three white 15-passenger Ford Transit 350 XL medium-roof vans based at 2120 N. Dixie Highway. Each van is branded with our logo and phone number, cleaned daily, inspected before each route, and staffed by screened, CPR-certified drivers who know every participant by name.
Final level and pricing depend on mobility, wheelchair logistics, distance from the facility, and route timing. Confirmed at assessment.

Mileage is not calculated from Boca Raton city center, ZIP-code center, neighborhood boundaries, county boundaries, straight-line radius, or the participant's physician/hospital. Every request is subject to exact address verification, actual driving mileage, route availability, mobility level, wheelchair/transfer requirements, vehicle capacity, scheduling, assessment, and final program approval. A city or ZIP code alone does not guarantee eligibility.
