Sundowning: Why It Happens and What Actually Helps

Sundowning — increased confusion, agitation, and restlessness in the late afternoon and early evening — affects up to 20% of dementia patients. The cause is not fully understood; circadian rhythm disruption, fatigue, low light, and unmet needs all play a role.

What helps: keep the home brightly lit through dusk, close blinds before it gets dark outside, maintain a predictable late-afternoon routine, limit caffeine and sugar after noon, schedule the most stimulating activities in the morning, and protect a quiet rest period in the early afternoon.

What makes it worse: arguing, asking 'don't you remember,' rushing the person, loud TV, and unfamiliar visitors at peak agitation time.

Structured daytime programming (like Program A) reduces sundowning significantly because the brain gets the right inputs at the right times.
